Filing An Identity Theft Complaint Form
With The FTC (Federal Trade Commission)

The FTC keeps a database of identity theft cases that is used by many law enforcement agencies.  If you have been a victim of Identity Theft, The FTC encourages you to file a complaint with them in one of three ways by:
  1. Filling out their online Identity Theft Complaint Form at:
    https://rn.ftc.gov/pls/dod/widtpubl$.startup?Z_ORG_CODE=PU03
  2. Calling the FTC's Identity Theft Hotline, toll-free:
    1-877-ID-THEFT (438-4338); TTY: 1-866-653-4261
  3. Writing to:
    Identity Theft Clearinghouse
    Federal Trade Commission
    600 Pennsylvania Avenue, NW
    Washington, DC 20580.

You can call the Hotline to update your complaint if you have any additional information or problems.

For more information on:

  • How to fill out the FTC Complaint Form
  • What you should know before filling out the form and
  • What you should do after filling out the form,

Go to this FTC webpage at http://www.ftc.gov/bcp/edu/microsites/idtheft/consumers/filing-a-report.html

By sharing your identity theft complaint with the FTC, you will provide important information that can help law enforcement officials across the nation track down identity thieves and stop them.

The FTC can refer victims' complaints to other government agencies and companies for further action, as well as investigate companies for violations of laws the agency enforces.

Create An Identity Theft Report

You can a print a copy of the FTC online form and give it to the police for them to incorporate it into their report. The printed FTC Identity Theft Complaint form along with a police report can constitute an Identity Theft Report and entitle you to certain protections.

This Identity Theft Report can be used to:

  1. Permanently block fraudulent information from appearing on your credit report
  2. Ensure that debts do not reappear on your credit report
  3. Prevent a company from continuing to collect debts that result from the identity theft
  4. Place an extended fraud alert on your credit report.

See the Topic on Filing an Identity Theft Report for more detailed information on creating this report when you file a police report..
